Abstract

Congenital heart disease (CHD) is a disorder both in the structure of the heart or large blood vessels and heart function that is obtained since it is still in the womb and can be a major cause of death in the first year of life. The prevalence of CHD events worldwide reaches 9.4 per 1000 live births including in Indonesia. Of these, around 300,000 cases are categorized as severe CHD which sometimes requires gradual surgery in order to survive. The purpose of this paper is to increase the understanding of the signs and symptoms of CHD by general practitioners with simple modalities that can be used in the initial screening of CHD in peripheral areas. Early detection of CHD in primary health services can be done with history taking, physical examination, and simple modalities such as pulse oximetry, electrocardiography, and chest X-ray. Early detection of CHD is expected to reduce the morbidity and mortality rate of CHD in Indonesia.

Abstract

Introduction: Myocardial perfusion scan (MPS) is a non-invasive procedure to evaluate the perfusion, viability, and contractility of the heart muscles. MPS is one of the important modalities using radionuclide in the management of heart disease. Single Photon Emission Computed Tomography (SPECT) is a technology applied to the MPS examination and can be combined with a CT scan.. Dextrocardia is an abnormality in the location and position of the heart, where the heart is located in the right hemithorax and accompanied by impaired perfusion. Case: We reported a 51-year-old woman, came with chest pain since 2 years. Chest pain appeared mainly with activity, radiates to the back, and sometimes with shortness of breath. Previous echocardiography had shown the heart was located in the right. CT scan examination gave inconclusive results. Patient was referred for an SPM examination on the indication of suspected coronary artery disease (CAD) with dextrocardia.Discussion: Patients with dextrocardia are at the same risk for developing myocardial ischemia as in the general population. SPM examination can be used to detect CAD in dextrocardia patients. Conclusion: SPECT/CT MPS could provide functional information and determine the type of dextrocardia in a patient suspected of having CAD.

Abstract

Coronavirus disease 2019 (COVID-19) is a global pandemic caused by Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) infection, affecting millions of people worldwide due to its ease of transmission. Despite limited information on effective therapeutic options, vitamin D has been regularly reported to exert beneficial immunomodulatory effects affecting both innate and adaptive immune systems. As it is synthesized in the skin under ultraviolet radiation, population living in equatorial countries are presumed to have adequate vitamin D, however several studies have shown otherwise. This article is aimed to give an insight on the different mechanisms by which vitamin D affects our immune system in COVID-19, as well as discussing correlation of having sunlight all year round by being near the equator towards vitamin D adequacy.

Abstract

Introduction: Gait analysis is the study about how to move. However, the normal gait reference which is currently used is the normal gait of Europeans. The body proportions, leg length, and height between Europeans and Indonesians are different. Methods: The samples were 44 Indonesian aged 18 – 26 years which was divided into 22 males and 22 females. Samples used legging and socks that had been marked with a color at a certain point. The research sample walked on a treadmill at low, medium and high speeds and recorded from the back and sides. Based on the video, the angles of the hip joint, knee join was assessed. Results: In male subjects, the joint angles of low, medium and high speed walking: maximal thigh joint (21.30o, 21.77o, 22.67o), minimal thigh joint (-7.05o, -7.74o, 8.66o), maximal knee joint (55.25o, 57.00o, 60.04o), minimal knee joints (9.63o, 10.13o, 10.50o). In female subjects, the joint angles of low, medium and high speed walking: maximal thigh joint (24.02o, 24.43o, 24.99o), minimum thigh joint (-3.25o, -3.86o, -4.22o), maximal knee joint (53.60o, 56.80o, 58.99o), minimal knee joints (11.12o, 12.94o, 12.83o).Conclusion: The faster walking speed, the greater angle of thigh joint and knee joint

Abstract

Introduction: Anosmia is one of the clinical characteristics of COVID-19 patients. Anosmia was reported as the first symptom before other symptoms were discovered. Prior studies have shown anosmia implications as a favorable prognostic factor in COVID-19 patients. The research aim is to describe the characteristics of COVID-19 patients with anosmia and its correlation with disease severity. Methods: This study was a descriptive, cross-sectional, retrospective study with a total sample of 149 patients confirmed COVID-19. This research was conducted on hospitalized patients from March 1 to July 31, 2020, at the Primasatya Husada Citra Hospital Surabaya. Our study classified patients into two groups, with and without anosmia. The patient’s clinical characteristics were described in a table and odds ratio was calculated to determine the risk factors incidence of severe degree in COVID-19 patients. Results: The mean age of patients with anosmia was 32 years old, the patient’s gender was dominated with the female (63.2%). Patients with anosmia had lower severity, the odds ratio of having severe degree is 0.479. Patients with anosmia also had other symptoms such as cough, rhinorrhea, sore throat, tightness, headache, and muscle aches (p <0.05).Conclusion: COVID-19 patients with anosmia had lower severity than patients without anosmia.

Abstract

Background: Ischemic stroke in Indonesia is one of the main causes of disability and mortality in elderly. Aging is related with the increase of comorbidities. The comorbidities in elderly increase the potential for polypharmacy prescribing which increases the incidence of drug side effects. This study’s purpose is to know the demographic characteristic, treatment pattern, and side effect of the treatment in elderly patients with ischemic stroke. Methods: This study is a descriptive study with a cross-sectional design which was conducted throughout September–October 2020. This study used secondary data from ischemic stroke patients’ medical records admitted in Dr. Hasan Sadikin Central General Hospital’s in 2015–2019, with simple random sampling.Result: Result of this study shows that most elderly patients with ischemic stroke were young elderly aged 60-69 years, male, with hypertension as the major risk factor. Polytherapy (2-4 drugs) was found in 79 patients and polypharmacy (>5 drugs) was found in 21 patients. Six patients with polytherapy and four patients with polypharmacy experienced drug side effects. Drug side effects of treatment observed were stress ulcer, upper gastrointestinal bleeding, and anemia. Conclusions: The treatment patterns were dominated by polytherapy (2-4 drugs). The percentage of elderly patients with polypharmacy (>5 drugs) experiencing side effects was more than those with polytherapy. Drug side effects were dominated by stress ulcer.

Method: The sampling technique used purposive sampling technique. This type of research is a descriptive study with a cross sectional survey design. The samples in this study were all active lecturers and students who filled out questionnaires through the Google Form application in July - December 2020. Result: The sample in this study amounted to 434 samples (58 lecturers and 376 students). The results showed that the information on COVID-19 was received quite effectively. This is indicated by the percentage of respondents' answers that 80,8% have received COVID-19 information and for the respondents' knowledge is good, which is shown by the percentage of respondents' answers, namely 87.5% who understand how to prevent and spread COVID-19. Conclusion: The percentage of receiving information and knowledge about COVID-19 by lecturers and students of Unimal is effective and good, this is indicated by 80,8% and 87.5% of respondents have received information and understand how to prevent COVID-19 at Unimal. This success is inseparable from the communication media used by the COVID-19 Task Force.
